I have been through mediation 

I have a question, I have been through mediationa and signed those papers, but my husband has refused to go sign the final papers? Also I recieved a letter from my attorny stating has sent a true and correct copy of the foregoing AMENDED ENTRY OF APPEARENCE. I dont know what that means?

I haven't started mediation yet, but maybe you could contact your mediator and have them clarify some of your questions. All I know is that it's important to stay on top of things.
