How do you remove ex from title/deed?

Hi, this is strictly a legal/procedural question.... the divorce papers are going to be filed next week, and it's an uncontested divorce through a mediator.  My soon-to-be-ex has agreed to come off the title on the house and cars, but I'm not sure exactly how to make that happen in terms of re-titling everything... if I understand right the divorce wouldn't make that happen automatically... I'd have to use the divorce papers to then remove her from the title, right?

Here's the three scenarios:
House: mortgage is only in my name, but she's on the deed
Car#1: loan only in my name, but she's on the title
Car#2: loan is joint, with both of us on the title

With Car#2, I'm going to be accepting full responsibility for the loan, though I won't be able to refinance the loan for about 6 months in order to remove her name from the loan.  Will I have to wait until then to remove her from the title?

So anyway, I'm guessing I go through the DMV for the car title stuff, but I don't know what all is required and I have no idea what the process is for removing her from the house.

quit claim on the deed - talk to your mortgage company, they can help you with that.

the cars - you have to refinance the one into your name before you can take her name off the title. The other, take your divorce papers to the DMV.

merlin, I think she will have to do a quit claim deed.
Not sure on the cars.

From Wikipedia:
An example of a circumstance where a quitclaim may be used is where one spouse (grantor) is disclaiming any interest in property that the other spouse (grantee) owns. A quitclaim deed would typically be used in this circumstance.
