He's signing my names to documents now

Hello I now seem to have another problem with him I just found a document which he signed my name to and somehow had it notorize the document was labled Exhibit #2 and in it state my name and that I was waiveing all rights and interest to the bank account (one of them there are a few) I never signed this I never would, my question is can this be used?? Isn't this illegal ?? he fordged my name is this something that will stand up in court?? Can he be charged with forgery and can I have the notary's licenses taken away for doing this they obviously did not witness me sign it (the notary is someone he knows) How do I prove I did not sign this, and if he did it with this I'm sure he signed my name to other documets stating I have no rights to anything , I need help does anyone know of a person who had this done and what the out come was ...I can't believe he did this what a deceitfull little man he is !! He doesn't know I know that he did this and I now have the paper in a safe deposit box at the bank ..So here I am again Very clueless, any advice what do I do.
